Conducting abortion research in fragile settings presents challenges, many of which are present in other low-resourced settings to various degrees but when appearing all together, collectively served to create a set of barriers to collecting data that required creative adaptations to address and even then, we could not overcome all of them.
RESULTS
Challenges that we experienced in the course of this mixed methods research project included limited access to the study sites by research team members, research being delayed to prioritize life-saving priorities which must take precedence when resource constraints mean that both cannot be carried out, a population skeptical of participating in research due to having negative experiences with the state/other actors as well as due to being research-naïve, geographic and language constraints impacting participant recruitment because of the fact that people are coming from various displaced locations to a particular health facility, a low literacy population meant that they could not read the consent form and due to the stigmatized subject matter we did not want a family member consenting them, and respondents’ challenges participating around the time of discharge because respondents needed to travel home with family members.
CONCLUSIONS
These strategies are relevant not only to abortion research but also other research in resource-constrained/fragile and conflict-affected contexts. Improving the health of the most vulnerable can only be done through understanding barriers to care in insecure and challenging environments. Recommendations include to plan for offsite and long-distance training, supervision, and quality assurance; attempt to negotiate flexible timelines with donors; hire field staff whose only responsibility is data collection; where possible, find a way to include the most vulnerable members of the study population; adapt informed consent processes for low literacy populations; and consider including travel support for respondents. Iterating improvements in data collection innovations in these contexts will advance the field by spurring more research upon which to base policy and practices.
INTRODUCTION
The health systems of countries in the South and the North have been directly affected by the COVID-19 pandemic. Healthcare workers have paid a high price. The aim of this study was to better describe the personnel who are on the front line when patients visit health care facilities and to analyze their risk factors for exposure, their perception of infection and the prevention practices implemented by these health care workers.
METHODS
A cross-sectional study was conducted over a 6-month period from October 2021 to March 2022 (i.e., at a distance from the index cases) among health care workers in 62 integrated health centers and five public urban hospitals in Niamey and Dosso. Information on socio-demographic characteristics, clinical symptoms, co-morbidities, knowledge and attitudes, and vaccination status was collected by means of a questionnaire. Blood samples were taken for serological analysis for each agent included.
RESULTS
A total of 733 agents were included, mostly women (628, 85.67%) with a mean age of 40.5 years. Only 5.5% (40/733) of the workers reported having been in contact with a positive case of Covid-19. The most common method of protection was the use of alcohol and soap for hand hygiene. 76% of them reported having been vaccinated against the SARS-CoV2 virus. However, only 7.7% reported having used a Covid-19 diagnostic test in the last few months. IgM and IgG COVID-19 serologies were positive in 1.2% and 92.2% of the health care workers, respectively, during the survey period.
CONCLUSION
In Niger, frontline health workers have been widely exposed to SARS CoV-2, but most of them don't think so. As a result, in their daily practice, they make poor use of means to prevent and control COVID-19 infection and rarely use diagnostic tests in case of illness. Vaccination was widely accepted by these staff, according to their statements.
The conflict in Syria has required humanitarian agencies to implement primary-level services for non-communicable diseases (NCDs) in Jordan, given the high NCD burden amongst Syrian refugees; and to integrate mental health and psychosocial support into NCD services given their comorbidity and treatment interactions. However, no studies have explored the mental health needs of Syrian NCD patients. This paper aims to examine the interaction between physical and mental health of patients with NCDs at a Médecins Sans Frontières (MSF) clinic in Irbid, Jordan, in the context of social suffering.
METHODS
This qualitative study involved sixteen semi-structured interviews with Syrian refugee and Jordanian patients and two focus groups with Syrian refugees attending MSF’s NCD services in Irbid, and eighteen semi-structured interviews with MSF clinical, managerial and administrative staff. These were conducted by research staff in August 2017 in Irbid, Amman and via Skype. Thematic analysis was used.
RESULTS
Respondents describe immense suffering and clearly perceived the interconnectedness of their physical wellbeing, mental health and social circumstances, in keeping with Kleinman’s theory of social suffering. There was a ‘disconnect’ between staff and patients’ perceptions of the potential role of the NCD and mental health service in alleviating this suffering. Possible explanations identified included respondent’s low expectations of the ability of the service to impact on the root causes of their suffering, normalisation of distress, the prevailing biomedical view of mental ill-health among national clinicians and patients, and humanitarian actors’ own cultural standpoints.
CONCLUSIONS
NCD patients recognised the psychological dimensions of their illness but may not utilize clinic-based humanitarian mental health and psychosocial support services. Humanitarian agencies must engage with NCD patients to elicit their needs and design culturally relevant services.
Infection with SARS-CoV-2 can lead to a detectable serological immune response even though extent of its protection is still not yet well known. We report long duration and resurgence of SARS-CoV-2 in patients with COVID-19.
METHODS
We included a cohort of 99 participants from our non-blinded non-randomized evaluation of COVID-19 tests in Cameroon. Demographic and clinical information was collected from participants including self-reported age, race, ethnicity, and gender. Qualitative data was described as proportions while quantitative data was described with means and accompanying ranges.
RESULTS
Duration of PCR for SARS-CoV-2 positivity was found to range from 4 – 81 days, with mean duration of 32.8 days in patients with PCR-positive. We also identified 4 participants who also had SARS-CoV-2 resurgence within a three-month period.
CONCLUSION
These observations raise questions on clinical decision to release COVID-19 cases from isolation after 14 days.
Abortion-related complications remain a main cause of maternal mortality. Post-abortion care (PAC) provision in humanitarian settings suffers from limited accessibility, poor quality, and lack of research on how to address these challenges. We proposed a comprehensive conceptual framework for measuring the quality of PAC and applied it using data from the AMoCo (Abortion-related Morbidity and mortality in fragile or Conflict-affected settings) study in two hospitals supported by Médecins Sans Frontières (MSF) in Jigawa State (Nigeria) and Bangui (Central African Republic, CAR).
METHODS
We adapted the WHO Maternal and Newborn Health quality-of-care framework to measure PAC inputs, process (provision and experience of care) and outcomes. We analyzed data from 4 study components: an assessment of the two hospitals’ PAC signal functions, a survey of the knowledge, attitudes, practices, and behavior of 140 and 84 clinicians providing PAC, a prospective review of the medical records of 520 and 548 women presenting for abortion complications, of whom 360 and 362 hospitalized women participated in an interview survey in the Nigerian and CAR hospitals, respectively.
RESULTS
Inputs – Among the total 27 PAC signal functions assessed, 25 were available in the Nigerian hospital and 26.5 in the CAR hospital.
Provision of care – In both hospitals, less than 2.5% were treated with dilatation and sharp curettage, a non-recommended technology when receiving instrumental uterine evacuation. Over 80% received blood transfusion or curative antibiotics when indicated. However, antibiotics were given to about 30% of patients without indication. All (99%) of discharged women in CAR received contraceptive counselling but only 39% in Nigeria.
Experience of care – Women reported generally good experience of respect and preservation of dignity. But only 49% in Nigeria and 59% in CAR said they were given explanations about their care and 15% felt capable of asking questions during treatment in both hospitals.
Outcomes – The risk of healthcare-related abortion-near-miss (happening =24h after presentation) was 0.2% in Nigeria and 1.1% in CAR. Only 65% of women in the Nigerian hospital and 34% in the CAR hospital reported that the staff provided them best care all the time.
CONCLUSION
Our proposed framework enabled comprehensive measurement of the quality of PAC in two MSF-supported hospitals in humanitarian settings. Its application identified that hospitals provided good clinical care resulting in a low risk of healthcare-related abortion-near-miss. However, hospitals need to improve provider-patient communication and would benefit from instituting antibiotic stewardships to prevent antibiotic-resistance.
